You must answer these questions if you are taking the class for a letter or S/U grade. This will allow us to get you computer accounts for the course and set up the course mailing list. The course mailing list will be used for urgent communications regarding the class, and your grade will suffer if you are not on the list. If you are auditing, you are not eligible for a computer account, and you should NOT fill in this form.
