If you have an exam conflict and need to petition for a makeup, submit TWO files to the appropriate CMS assignment by two weeks before the exam: (1) a copy of this file with all the requested info filled in, in plaintext format; see below for formatting instructions. It's OK to leave in the instructions written here. (2) a screenshot of your weekly class schedule from student center (not required for final-exam conflicts). See CMS for allowed file formats (.pdf? .png? etc.), and see below for instructions on creating screenshots. If an emergency comes up after the petition deadline, please still submit to CMS if you can, but most importantly, contact the instructor(s), head TA, and course administrative assistant (See "Staff" page at http://www.cs.cornell.edu/courses/cs1110) ................ YOUR FIRST NAME: YOUR LAST NAME: YOUR NETID: YOUR TELEPHONE NUMBER: (We would only call it during business hours, and only as a last resort; we've had some problems with emails getting through on occasion, and want to prevent such communication failures from happening again.) NAME OF COURSE WITH CONFLICTING EXAMS (E.G., HADM 2220): NAME(S) OF THE INSTRUCTOR OF SAID COURSE(S): EMAIL ADDRESS OF SAID INSTRUCTOR(S): WHEN ARE THE CONFLICTING CLASSES' MAKEUPS SCHEDULED FOR?: OTHER KIND OF CONFLICT (explain in detail. If you will be out of town, include the date and time you are leaving and the date and time you are returning): ARE YOU AVAILABLE 8-9:30AM THE MORNING AFTER THE EXAM? (leave blank if the regular exam is on a Friday) IF THE REGULAR EXAM IS A FINAL EXAM: LIST *ALL* DATES, TIMES, COURSE NAMES FOR FINALS YOU ARE SCHEDULED FOR (there's no point in our scheduling a makeup on top of some other exam you have): ANY OTHER RELEVANT INFO: ................ How to save a file in plain text format if you use Word to edit this template: Use the "Save as..." feature (most likely located in the "File" menu item), and choose plain-text (.txt) as the format. ................ How to create a screenshot of your class schedule (written by Qin Jia and Caleb Perkins): Go to the class schedule page of the student center in your browser: Type studentcenter.cornell.edu in your browser, log in with your netId, click on “My Class Schedule” on top of the page and then choose the current semester. Windows: 1. Press "PrtSc" on your keyboard. Mostly, it will be near the right top of the keyboard and near the keys "ScrLk" and "Pause". 2. Open "Paint" application from your start menu. 3. Right click and choose "paste" (or just press Ctrl + V) in Paint. You can see that the screenshot is already in your paint. 4. Save the picture show in your Paint. Mac: 1. Press Command-Shift-3, which saves a screenshot to your Desktop.
